WHAT HAPPENS NEXT?
Your FREEDOM Wellbeing Institute accredited Facilitator will
a) send your Universal Wellbeing Evaluation Tool (UWET) Report to you and
b) offer to arrange a meeting with you to go over your UWET Results.
REMEMBER
The information in your UWET Report is Confidential, only you and your
Accredited Universal Wellbeing Facilitator can see it.
Your UWET Report cannot be disclosed to any other person without your written permission.
IMPORTANT
You were asked to give your Consent to having your UWET results included in research we are conducting
(without you being identified) at the end of the UWET.
If you have not yet said YES you may still do so, just let your Facilitator know.
Ethically approved Research Information Sheet and Consents for Adults and Minors
(those under 18 years) can be opened below for you to read and or download.
